  4. Must have a GENERAL AVERAGE of 80%, no grade below 80 in final rating in all subjects.
  5. Official Receipt of Testing Fee – 125.00 Php (Non Refundable)

Grade Eleven

  1. Report Card
  2. Certificate of Good Moral Character
  3. 2 pcs. 2×2 Colored Recent picture (Not Cut-out)
  4. Must have a GENERAL AVERAGE of 80%, no grade below 80 in final rating in all subjects.
  5. Official Receipt of Testing Fee – 125.00 Php (Non Refundable)
  6. NCAE Result
  7. Senior High Preference Slip

College

  1. Report card
  2. Certificate of Good Moral Character/Brgy. Clearance
  3. 2 pcs. 2×2 Color Recent Picture (Not Cut-out)
  4. Official Receipt of Testing Fee (125.00 Php) (Non Refundable)
  5. Must be physically fit
  6. For Teacher Education applicants: the General Average is at least 85% or 2.0 for transferees (No rounding off). No grade below 80 in the final rating in all subjects
  7. For other degree courses GENERAL AVERAGE is at least 80%
  8. Graduates of Non-Degree courses who wish to proceed to Degree Courses must have at least 80% or 2.5 General Average of the course last attended.
